Meniscal root tears compromise knee joint biomechanics and are associated with early-onset osteoarthritis. This study introduces a novel self-tension screw designed to enhance meniscal root repair by providing controlled tensioning during fixation. Biomechanical testing demonstrated that the self-tension screw offers improved load distribution and stability compared to conventional fixation methods. The design considerations focus on optimizing mechanical properties to mimic native meniscal function, potentially leading to better clinical outcomes.

Falls among older adults (individuals aged 60 and above) are a substantial health issue worldwide. This study aimed to analyze the burden of falls and its attributable risk factors among older adults at the national and subnational levels in Iran over 32 years. Using the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2021 data, we estimated the incidence, prevalence, death, and disability-adjusted life-years (DALYs) of falls and its attributable risk factors among older adults by sex, age groups, and socio-demographic index (SDI) in Iran and its provinces. We reported the estimates with their 95% uncertainty intervals (UIs). Rates were reported per 100,000 population. In 2021 in Iran, the incidence rate of falls among older adults was 1674.0 (95% UI: 1454.9-1897.3), the prevalence rate was 11302.5 (10504.7-12095.7), the death rate was 16.9 (12.9–21.0), and the DALYs rate was 736.3 (647.6-825.4). In 2021, at the subnational level, Qazvin had the highest incidence, death, and DALYs rates for falls with values at 2329.5 (2008.8-2652.1), 24.2 (19.5–29.0), and 965.9 (856.2-1074.6), respectively, while Kohgiluyeh and Boyer-Ahmad had the highest falls prevalence rate at 16043.1 (14918.4-17149.0). In 2021, males had higher prevalence, death, and DALYs rates of falls compared to females, while females had a higher incidence rate. Among the age groups, the 90–94 age group had the highest rates of incidence, prevalence, death, and DALYs from falls. Low bone mineral density was the primary risk factor attributable to the burden of falls. There were significant positive associations between SDI and both the incidence and prevalence rates of falls. Conversely, a significant inverse association was found between SDI and the death rate. From 1990 to 2021, the incidence rate of falls has increased significantly among older adults in Iran, necessitating urgent interventions. Implementing nationwide, cost-effective strategies such as exercise programs to improve strength and balance, home hazard modifications, medication reviews to reduce fall-related risks, and routine screening programs for osteoporosis and fall risk assessment can help protect and support older people, minimizing their risk of falls

Atrial fibrillation/flutter (AF/AFL) is one of the most common sustained heart rhythm disorders in clinical practice and a major public health concern. This study aimed to evaluate the disease burden of AF/AFL in Iran and analyze trends using the Global Burden of Disease (GBD) 2021 data, by age, sex, location, risk factor, and socio-demographic index (SDI), considering the impacts of COVID-19. Data on the prevalence, incidence, disability-adjusted life years (DALYs), deaths, and six attributable risk factors related to AF/AFL in Iran and its 31 provinces from 1990 to 2021 was collected from the GBD 2021 study. The International Classification of Disease (ICD) codes used were I48–48.9 for ICD-10 and 427.3 for ICD-9. The data was sourced from surveys, censuses, vital statistics, and other health-related records. In Iran, in 2021, the AF/AFL age-standardized incidence rate (ASIR) was 40.6 (30.0 to 54.4), the age-standardized prevalence rate (ASPR) was 425.4 (327.2 to 559.2), the age-standardized DALY rate was 72.4 (57.4 to 88.3), and the age-standardized death rate (ASDR) was 3.3 (2.5 to 3.8) per 100,000 population. Following COVID-19, there were significant decreases in age-standardized DALY and death rates over 2019–2021. By province, Fars had the highest AF/AFL ASIR and ASPR in 2021. In addition, East and West Azarbayejan had the highest age-standardized DALY rate and ASDR, respectively. The national incidence, prevalence, DALYs, and death rates of AF/AFL showed an overall increasing trend with age. Males experienced higher rates of incidence and prevalence compared to females. In contrast, females had higher rates of DALYs and deaths compared to males. The burden of AF/AFL increased with advancing age. The risk factor with the highest DALYs and deaths attributable to AF/AFL was high systolic blood pressure. Notably, no remarkable association was found between SDI and the burden of AF/AFL in Iran. The incidence and prevalence of AF/AFL in Iran have significantly increased, highlighting the critical need for cost-effective and nationwide interventions.

Neural tube defects (NTDs) represent a significant burden on global pediatric health, contributing to high rates of infant mortality and morbidity. Despite extensive research into their etiology, NTDs continue to pose challenges in diagnosis and treatment. MicroRNAs (miRNAs) have emerged as promising candidates for understanding the molecular mechanisms underlying NTDs and potentially offering avenues for improved diagnosis and therapeutic intervention. This review explores the multifaceted roles of miRNAs in the context of NTD pathogenesis. Studies have identified specific miRNA profiles associated with NTDs, providing insights into their potential as diagnostic biomarkers. Furthermore, dysregulation of certain miRNAs has been implicated in the pathophysiology of NTDs, highlighting their role as potential therapeutic targets. Additionally, animal models and deep sequencing approaches have expanded our understanding of the diverse miRNA expression patterns associated with NTDs. By unraveling the intricate molecular mechanisms underlying NTD pathogenesis, miRNAs offer promising avenues for early detection and intervention, ultimately improving outcomes for affected individuals.

Lateral Ankle sprain is a common sports injury with a high incidence rate after muscle fatigue. Health specialists are searching for the best conservative approach to reduce the complications of this injury and decrease the incidence rate after fatigue. This systematic review and meta-analysis aimed to assess the effectiveness of external ankle support on balance in patients with ankle instability following muscle fatigue. We conducted a systematic search of PubMed, Scopus, and Web of Science up to April 29, 2024, to find articles involving populations with ankle instability, interventions using external ankle support (taping or ankle orthoses), and outcomes related to balance assessed during muscle fatigue. The Risk of Bias 2 tool was used for risk of bias assessment. Data on patients, interventions, fatigue protocols, and outcomes were extracted and analysed. A meta-analysis was performed using Review Manager 5.3 software, calculating standardized mean differences with 95% confidence intervals (CI) for each outcome. Nine crossover studies involving 230 subjects were included. The narrative synthesis shows that external ankle supports improve the single leg hop test [Mean difference=7.84, P=0.01], center of pressure range and velocity, and ankle inversion. The meta-analysis results indicate that compared to the control group, external ankle supports after muscle fatigue significantly change Y balance clinical test-posterolateral [MD=1.22, 95% CI (0.16, 2.28), p=0.03], Y balance clinical test-posteromedial [Mean difference=2.60, 95% CI (0.42, 4.79), p=0.03], center of pressure time to stabilization-mediolateral [Mean difference=-0.68, 95% CI (-1.29, -0.07), p=0.04], center of pressure time to stabilization-vertical [Mean difference=-0.62, 95% CI (-0.94, -0.30), p=0.01], and vertical ground reaction force [Mean difference=0.58, 95% CI (0.21, 0.96), p=0.02]. External ankle supports are an effective conservative intervention for improving specific aspects of balance in patients with ankle instability, particularly following muscle fatigue. Significant improvements were observed in the Y Balance Test, center of pressure measures, vertical ground reaction force, and single-leg hop performance. Healthcare professionals are encouraged to incorporate external ankle supports into rehabilitation programs for individuals with ankle instability. These supports can enhance balance, improve functional outcomes, and help mitigate the negative effects of muscle fatigue on postural stability.

Bacterial gastroenteritis is a significant public health concern, capable of causing severe infections. Among the various pathogens involved, those belonging to the Enterobacteriaceae family are the most frequently isolated and associated with gastrointestinal disorders. This study aimed to investigate the prevalence of common diarrheagenic Enterobacteriaceae in Iran over the past two decades, from 2000 to 2023. A comprehensive systematic search was conducted across multiple databases, including EMBASE, HINARI, MEDLINE, PubMed, Google Scholar, and the Cochrane Library. The focus was on observational published studies reporting the prevalence of diarrheagenic Enterobacteriaceae in Iran during 2000 and 2023. The criteria did not restrict patient demographics such as age, gender, health conditions, or occupation. This meta-analysis employed a 95% confidence interval (CI) for analysis. Evidence of heterogeneity was determined using an I² value greater than 50%. To explore potential sources of heterogeneity, subgroup analysis and meta-regression analysis were performed. Statistical analyses were executed using R version 4.3.2 along with the meta package. A p-value less than 0.05 was considered statistically significant. Out of 3,701 papers reviewed, 56 studies met the inclusion criteria and were analyzed. The overall pooled prevalence of diarrheagenic Enterobacteriaceae species from 2000 to 2023 was 14.0% (95% CI: 0.11–0.17). Subgroup analysis revealed Shigella spp. had the highest prevalence at 18.0% (95% CI: 0.13–0.24; I²=99%), followed by diarrheagenic Escherichia coli at 11.0% (95% CI: 0.09–0.15; I²=97%), Salmonella spp. at 9.0% (95% CI: 0.05–0.17; I²=99%), and Yersinia spp. at 2.0% (95% CI: 0.00-0.10; I²=94%). Prevalence trends showed Shigella spp. increasing from 4% (95% CI: 0.03–0.08) in 2000–2004 to 36% (95% CI: 0.20–0.55) in 2021–2023. Diarrheagenic E. coli (DEC) showed no clear pattern, ranging from 5% (95% CI: 0.01–0.24) to 17% (95% CI: 0.07–0.36). Salmonella spp. exhibited more significant fluctuations, rising from 6% (95% CI) in both 2000–2004 and 2005–2008 periods to 20% (95% CI: 0.03–0.66) in 2009–2012 and 30% (95% CI: 0.11–0.60) in 2017–2020. Yersinia spp. was only determined in 2000–2004 with a pool prevalence of 12% (95% CI: 0.00-0.91). Sub-species analysis revealed Shigella sonnei was the most prevalent species among Shigella spp. in Iran, accounting for 42% of cases (95% CI: 0.33–0.52). Regarding DEC species, Enteroaggregative E. coli (EAEC) and Enterotpathogenic E. coli (EPEC) had the highest rate at 15% (95% CI). Furthermore, a pool prevalence of 2% (95% CI: 0.00-0.89) was reported for Yersinia enterocolitica among diarrheagenic Enterobacteriaceae in Iran. This meta-analysis provides valuable insights into the prevalence of diarrheagenic Enterobacteriaceae in Iran over the past two decades. The findings highlight the significant impact of these pathogens on public health, with Shigella spp. showing the highest prevalence and increasing trends. Further research should investigate the factors contributing to the prevalence of diarrheagenic Enterobacteriaceae, including genetic diversity of diarrheagenic Enterobacteriaceae isolates, molecular mechanisms underlying the virulence of these pathogens, or antibiotic resistance patterns.

Congenital idiopathic clubfoot is a significant musculoskeletal deformity affecting children globally. It is characterized by hindfoot varus, midfoot cavus, forefoot adduction, and ankle equinus. The Ponseti method has proven to be highly effective and is now the gold standard for treatment. However, long-term follow-up studies indicate a variable relapse rate. Timely diagnosis of relapse is crucial, underscoring the importance of regular follow-up visits. Poor compliance with brace wear is a primary cause of relapse; therefore, educating families on proper adherence to the treatment regimen is essential. This review article discusses the anatomy, etiology, various treatment approaches, and strategies for managing relapses in clubfoot.

📄 Abstract:
Osteoarthritis (OA) affects weight-bearing joints, such as hips and knees, and its prevalence is rising due to factors like obesity and aging. Muscle atrophy, exacerbated by aging and surgery, increases the risk of joint instability and falls. Orthopedic surgeons explore dietary interventions to counteract these effects, with protein supplementation (PS) showing promise. This systematic review and meta-analysis assessed the effectiveness of PS in arthroplasty patients, comparing findings with sports medicine and sarcopenia literature. Following PRISMA guidelines, we searched PubMed, Web of Science, Scopus, and Embase (February 2025) for protein and amino acid supplementation studies in total knee or hip arthroplasty (TKA and THA) patients. The quality assessment used the Cochrane risk of bias and the Newcastle–Ottawa Scale. Meta-analysis calculated effect sizes for muscle atrophy and strength outcomes. Nineteen studies (903 patients) evaluated oral or intravenous protein/amino acid supplementation over a mean follow-up of 55.2 days. Essential amino acids (EAA) significantly reduced muscle atrophy in quadriceps femoris muscle mass (SMD: 0.69; 95% CI: 0.44 to 0.95) and hamstring muscle mass (SMD: 1.04; 95% CI: 0.52 to 1.55). However, effects on intramuscular adipose tissue (IMAT) and muscle thickness (MT) were inconsistent. Muscle strength outcomes varied, with no significant effect on quadriceps muscle strength (QMS) or handgrip strength (HGS). Intravenous amino acid infusion improved muscle protein synthesis and reduced perioperative blood loss. Protein and amino acid supplementation can reduce muscle atrophy in hip or knee arthroplasty patients. While effects on muscle strength and function are mixed, intravenous supplementation offers benefits. Further standardized research is needed to confirm these findings.

The burden of orthopedic admissions has notably increased in recent years. Managing orthopedic conditions is challenging in clinical settings. Orthopedic complaints often necessitate urgent medical intervention to prevent complications and mortality. Despite advancements in medical care, some patients still experience severe complications, extended hospital stays, and death following orthopedic admission. In this study, we aimed to explore the distribution of potential risk factors and common patterns in orthopedic patients who died during their hospitalization. All the patients who were admitted to three tertiary trauma centers with orthopedic complaints from 2010 to 2023 and died during hospitalization were enrolled in this study. Demographic, injury-related, laboratory-related, intervention-related, complication-related, and healthcare-related data were extracted using the patient’s medical records. Descriptive analysis of the collected data was performed using the SPSS version 27 software. 333 patients who died in the hospital with orthopedic complaints were included in the study and examined. The mean age of patients in this study was 67.89 years, comprising 68% males and 32% females. Trauma was patients’ most common clinical cause of admission (63.7%). The prevalence of death before surgery, death during the first 24 h after surgery, and death after 24 h postoperatively were 26.4%, 18.6%, and 55%, respectively. Our findings suggest a high prevalence of trauma as a clinical complaint leading to death among patients, emphasizing the importance of developing an integrated protocol for trauma preventive strategies.

Physiological storage temperature and chondrogenic supplements can enhance tissue viability, potentially overcoming the limitations associated with osteochondral allograft transplantation. This study aimed to evaluate the quality of macroscopically healthy cartilage across 12 different storage protocols to find optimum storage conditions for osteochondral allograft plugs. Osteochondral plugs were obtained from arthroplasty candidates and divided into 12 groups based on two culture media (Media 1 [supplemented Dulbecco's modified Eagle's medium {DMEM}/Ham's Nutrient Mixture F12] or Media 2 [enriched Media 1 with 10 ng/mL of transforming growth factor-beta {TGF-β}]), two culture conditions (static or dynamic), and three temperatures (−70°C, 4°C and 37°C). Subsequently, samples were evaluated on Days 1, 14, 28 and 60 for biochemical, biomechanical and histopathological characteristics alongside bacterial surveillance. A total of 4338 plugs from 843 donors were assessed. Chondrocyte viability and proteoglycan synthesis were highest in the DMEM enriched with TGF-β at 37°C and 4°C. Although biomechanical properties decreased over time in all groups, dynamic culture conditions resulted in smaller decreases compared to other storage protocols. Viscoelasticity was observed in all samples, with dynamic media groups being maintained the most. Histological evaluation showed signs of degeneration, and temperature variations affected the preservation of the tissue differently. Bacterial surveillance identified contamination in specific storage conditions. Storing osteochondral allografts at 37°C in TGF-β supplemented media under dynamic conditions may extend the grafting window from 14 to 60 days. This extension could improve tissue availability, reduce costs and minimize graft wastage, thereby advancing joint resurfacing techniques. Further research is needed to confirm the safety and efficacy of this storage protocol.

Effective management of bone defects is crucial in revision total knee arthroplasty (rTKA) to ensure implant stability in low-density bone environments, especially following prosthesis removal. While various reinforcement techniques exist, such as allografts, augments, metal cones, sleeves, or combinations thereof, metal cones are increasingly favored for their ability to reconstruct significant bone defects. A prominent challenge with off-the-shelf (OTS) cones is their unsuitability for Asian and specifically Iranian anatomies due to mismatched sizes and smaller medullary canals. This study explores the application of customized 3D-printed porous titanium cones, tailored to individual patient anatomy, for bone defect reconstruction in complex primary or revision total knee arthroplasty (TKA). Utilizing CT scans for 3D preoperative planning, porous cones were individually designed to fit each patient’s anatomy. A novel aspect of these cones is their dual functionality: promoting bone ingrowth while preventing cement infiltration. The cones were fabricated using selective laser melting with medical-grade Ti6Al4V alloy. To aid in surgical preparation, single-use polyamide trials were also 3D-printed. A total of nineteen cones/half cones (four diaphyseal, twelve metaphyseal, and three meta-diaphyseal) for femoral and tibial applications were successfully designed and implanted across ten cases. There was a perfect alignment between preoperative planning and intraoperative outcomes, with precise defect reconstruction achieved in all cases. Early follow-ups indicated no loosening or implant migration. Customized patient-specific porous cones offer not only the long-term benefits of OTS options, such as osseointegration and biological fixation but also accurately address a wide range of complex metaphyseal and diaphyseal defects. This approach preserves residual bone stock and streamlines the surgical process, presenting a promising advancement in bone defect management in rTKA.

In patients with cerebral palsy (CP), spastic hip is a prevalent complication. Various surgical approaches,, including pelvic osteotomy (PO), femoral osteotomy (FO), combined femoral and pelvic osteotomy (CFPO), and soft tissue surgery (STS), have been used to address this problem. This systematic review and meta-analysis was designed to compare the radiologic outcomes of these interventions for spastic hip in patients with CP. To identify relevant studies, databases were searched using specific keywords. Initially, duplicates were removed, then the titles and abstracts were screened, followed by a comprehensive full-text review. Data extraction took place from the studies that met the inclusion criteria. Subsequently, a meta-analysis was conducted. The analysis of 6116 hips from 4546 patients across 81 studies demonstrated that PO significantly enhanced the center–edge angle (CEA), reduced the acetabular index (AI) and migration percentage (MP), and improved the Sharp and Tönnis angles. FO led to a substantial decrease in AI and MP, though CEA did not show a significant change, while CFPO resulted in significant improvements across AI, MP, neck-shaft angle (NSA), CEA, Sharp angle, and Tönnis angle. STS did not show significant changes in AI or CEA, but MP was notably reduced. Tone-decreasing procedures, such as selective dorsal rhizotomy and botulinum toxin injections, did not significantly alter MP, whereas guided growth techniques showed a significant reduction. MP improvements in FO decreased over time, with other radiologic parameters remaining relatively stable as follow-up increased. Age-specific trends indicated that children under 6 years primarily underwent tone-decreasing procedures and STS, while those around 7 years favored FO and guided growth, and older children (over 9 years) more commonly underwent PO, CFPO, or percutaneous osteotomy. Comparative analysis showed PO and percutaneous osteotomy were particularly more effective in reducing MP, with PO also being superior for AI improvement; whereas CFPO provided better outcomes for enhancing CEA. No significant differences were found among surgical methods for improving NSA. This systematic review and meta-analysis underscores the superior efficacy of PO and CFPO in correcting spastic hip deformity in children with CP. Radiological outcomes demonstrate significant improvements following these procedures. The findings suggest that these approaches are particularly effective for complex cases where procedures such as FO, STS, or TDS may fall short. Future studies should focus on refining surgical protocols and exploring the long-term functional outcomes of these interventions.

The aim of this study was to investigate the efficacy of our modified cementing technique in reducing the rate of aseptic tibial loosening focusing on its relationship with keel length. Every participant who underwent primary total knee arthroplasty (TKA) between August 2014 and September 2022 with a minimum of 4-year follow-up using 1 of 3 implants were included: Persona + conventional cementing technique; Persona + modified cementing technique; and NexGen LPS-Flex. The modifications applied include better preparation of the bone surfaces and the cancellous bone cavities, pressurizing the cement and interstitial fluid suction at the same time, applying a layer of cement on the surfaces of the tibia and implant, and immobilizing the limb. Kaplan-Meier analyses were performed to estimate survivorship. A total of 988 of 1039 primary TKAs (95.1%) were included with follow-up of 89.26 ± 7.32 months. Twenty eight (2.83%) TKA required revisions due to aseptic tibial loosening; 3 (1.1%) in the NexGen group, 21 (6.9%) in the conventionally cemented Persona group, and 4 (0.9%) within the modified cemented Persona group. Aseptic loosening occurred at a mean of 69.00 ± 2.65, 34.57 ± 22.90, and 68.50 ± 3.42, respectively. Survivorship for aseptic loosening was 98.9%, 93.1%, and 99.1% at 8 years, respectively. The revision rate for early (during the first 24 months) aseptic loosening was 4.6% in the conventionally cemented Persona group. No early aseptic loosening reported in other 2 groups. In conclusion, meticulous cementing techniques can reduce the rate of tibial loosening in shorter keel designs.
