💊: My name is Aspirin, but my scientific family name is Acetylsalicylic Acid (ASA).
👨🏻⚕️: What is your work?
💊: I am a Pain reliever (Analgesic), Fever reducer (Antipyretic), Anti-inflammatory, and Antiplatelet agent.
👨🏻⚕️: Tell me about yourself.
💊: I belong to the Non-Steroidal Anti-Inflammatory Drug (NSAID) class. My origins trace back to willow bark, but I was first synthesized in a pure form in 1897 and introduced to the world by Bayer.
👨🏻⚕️: What is your mechanism of action?
💊: My primary action is the irreversible inhibition of the Cyclooxygenase (COX) enzymes, both COX-1 and COX-2. By doing this, I block the production of prostaglandins and thromboxanes, which are responsible for pain, fever, inflammation, and platelet aggregation. My antiplatelet effect is unique because it lasts for the entire lifespan of the platelet (7-10 days).
👨🏻⚕️: What about the metabolism of you?
💊: I am rapidly hydrolyzed into salicylic acid in the gut wall and liver. Salicylic acid is then further metabolized in the liver by enzymes such as CYP2C19.
👨🏻⚕️: What about your elimination?
💊: My metabolites are primarily excreted in the urine (80-100%). The elimination half-life depends on the dose, ranging from 2-3 hours for low doses to 15-30 hours for higher doses.
👨🏻⚕️: How is Aspirin administration?
💊: This is a key point for safety and efficacy.
🔻 I am given orally, with a full glass of water, and can be taken with food to reduce stomach upset.
🔻 For antiplatelet effects, low doses (e.g., 75-100 mg) are used. For pain or fever, higher doses (e.g., 325-650 mg) are used.
🔻 Chewable tablets must be chewed, and enteric-coated tablets must be swallowed whole.
🔻 Do not use me if I have a strong vinegar smell, as this means I have decomposed and am no longer effective.
